Cancellation Policy

Appointment times represent time set aside by Dr. Koffler to address your specific medical and vision needs. The care of our patients is the primary goal of Koffler Vision Group. Due to an increase in patient cancellations and no shows for same day appointments, we are having difficulty seeing the patients that need our care.

Therefore, our office is regrettably instituting a new policy. A charge of $35.00 will be billed to the patient when a scheduled appointment is not cancelled 24 hours in advance. This fee is not covered by your insurance company and payment is the responsibility of the patient.

Please be courteous enough to cancel your appointment 48 hours in advance to allow adequate time for us to reschedule another patient in your time slot.

Please print and sign the cancellation policy form to indicate your understanding of this policy and bring it with you to your appointment.

Prescription Requests/Refills Policy

Due to the increased time necessary to refill prescriptions, we are now requiring 48 hours notice to process any requests. If your insurance requires prior authorization, your prescription will take an average of 4 to 5 days to complete. Please do not wait until you have no medication left to call and ask for a refill.

We are sorry for any inconvenience this may cause. All of our patients are incredibly important to us and our staff will address all of your concerns as soon as possible.
